EyeMed Vision Care
EyeMed is a voluntary vision care benefit offered to full-time employees of Hamilton
County Department of Education as of January 1, 2009.
EyeMed serves over 143 million
members in the U.S. and offers a wide network of independent and national retail
providers including LensCrafters, Pearle Vision, Target Optical, Sears Optical,
J.C. Penney Optical and Sunglass Hut.
Vision care is essential for maintaining overall
good health. Eye exams not only identify vision related problems but may also detect
other major medical conditions that affect our health such as diabetes, high blood
pressure, heart disease and more.
Members pay $10 copay for an annual eye exam and
$15 copay for the lenses. Frames are covered at 100% up to a $120 allowance with
a 20% discount for balances over $120.* Please refer to the plan design document
for further information on the benefits and frequency of coverage.

the policy number 9728981 and specify that the network is the “Select Network”.
To check for participating providers near you, go to the EyeMed website at www.eyemedvisioncare.com
and choose the “Select Network” from the provider locator drop down box and enter
your zip code.
Premium for the vision plan is payroll deducted on a pre-tax basis.
